I look forward to seeing you on your scheduled appointment date. My office may contact you a few days prior to your appointment to confirm the appointment date and time, pre-register standard information and remind you of the upcoming appointment. When checking-in, it may be necessary to obtain copies of insurance ID card(s) in order to bill your insurance company.

Appointment Scheduling, Cancellations and Communication
Free 15 minute Consultations
These free initial visits are available by phone. This consultation allows us to briefly get to know each other, your mental health needs and services I can offer. This free service does not guarantee future care, and does not establish a client/provider relationship. If the we both agree to work together, a full intake assessment will be scheduled. Following the free consultation, a full intake appointment can be scheduled either by the provider or patient via the patient portal. The frequency of follow up visits will be determined at a cadence we agree on. It depends on factors such as the severity and complexity of your situation, current symptoms, the types of medications prescribed, and the availability of additional support from sources like family, friends, or therapists. Visits may range from monthly or more frequently initially, until you achieve stability and improved well-being, to quarterly visits once all necessary reports and assessments are up-to-date. In line with widely accepted community care standards, clients receiving psychotropic mediations must be evaluated by a medical professional at least once every 90 days to continue their medication regimen. Failure to attend an evaluation within this timeframe may require a follow-up visit to assess medication needs.
Payment
A current form of payment, either debit or credit card, must be placed on file to schedule your appointment. If you utilize health insurance, Headway will process your payments. If you are a self-pay client your payment will be processed via Stripe in the patient portal.
Late Arrival Policy
Arriving late (more than 10 minutes) for an appointment may lead to cancellation or rescheduling.
Contacting Me
I am often not immediately available by telephone during business hours as I do not answer my phone when I am with clients or otherwise unavailable. During those times, you may leave a message on my confidential voicemail and I will return your call once I’ve reviewed your chart, but it may take a day or two for non-urgent matters. I check voice mail messages regularly during normal business hours. Messages left outside of normal hours of operation will be picked up the next business day.
After-Hours Emergencies
Please be aware I do not provide crisis management services. If you are experiencing suicidal or homicidal thoughts, are in crisis, or need immediate help, please call 911 or go to the nearest emergency department. I do not follow patients in the hospital, or see patients while hospitalized. When hospitalization should occur/become necessary, the inpatient treatment facility or hospital typically provides an in-patient psychiatrist. Once you have discharged from your higher level of care, whether it be inpatient, partial hospitalization, intensive outpatient program or residential care, please make an appointment with my practice for continuation of treatment.
Cancellation or No-Show Fee
I kindly request a minimum of 24-hour notice for appointment cancellations or rescheduling. This allows me to allocate the appointment to someone on my waitlist, ensuring efficient care for all. Clients who cancel an appointment or fail to attend with less than 24 hours notice are subject to a $90 cancellation or no-show fee, this applies to both new patients and established patients. Please be aware that you hold financial responsibility for appointments not rescheduled or canceled with appropriate advance notice.
Provider Cancellations
I will make every attempt to inform you in advance of planned absences, and provide you with the name and phone number of the mental health professional covering my practice. If I need to cancel an appointment at the last-minute, I will reach out as soon as possible and reschedule, or have a member of my staff connect with you.
Emergency Circumstances
If an emergency necessitates a change to your appointment, please contact my office promptly through my patient portal or by calling 817-484-1810 and leaving a voicemail. I strive to accommodate your needs, although immediate rescheduling of missed appointments cannot be guaranteed.
Missed Appointments
Repeated instances of missed appointments without prior notification may result in your discharge from my practice. This decision aims to ensure that I can offer care to all patients effectively.
